AEMT Clinical Documentation

To successfully complete your clinical requirement, you will need to submit
ALL** of the following.

  1. Documentation of the time you were at your clinical site. You must have this filled out every time you are at a clinical site. Any clinical documentation that does correspond with a signed clinical shift will not be honored.

  2. Documentation of at least 25 successful IV starts. This can include saline locks. No more than 10 may be acquired in the classroom lab setting.

  3. Documentation of at least 20 bag-mask ventilations. All 20 may be acquired in the classroom setting on an airway mannequin.

  4. Documentation of at least 15 medication administrations. All must be in the clinical setting under the direct supervision of a preceptor. IV solutions and Oxygen administration cannot be counted towards this requirement.

  5. Documentation of competent patient assessments for the following categories: Adult, Geriatric, Cardiac, Respiratory, Pediatric & Altered Mental Status.

You cannot use one patient to satisfy multiple categories. For example. You can not have a 78-year-old with chest pain and use that same patient for Adult, Geriatric, and Cardiac. You must choose only one category for that patient.

Each PCR must have a student evaluation attached to it. This must be scored by your preceptor. If you score less than 3 or any category, you must complete a new, different assessment on a new patient. PCRs submitted without a student evaluation will not be accepted.
