Please include in your email:

1. Your Full Name
2. Your email address
3. Your phone number
4. Indicate you are with the NHCPR program
5. Indicate what level your program is (AEMT)

Give a list of what days and times you are available. Be sure to add as many days as possible as this will allow Concord Hospital greater flexibility as their EMS student schedule may be very full.
